Class registration form template for kids classes

The checklist, part 1

What should a class registration form include?

A class registration form for kids classes should collect the student's name and age or grade, the class and session they're joining, parent or guardian contacts, an emergency contact, medical and allergy notes, pickup authorization, a signed waiver, a photo release and payment. A paper sign up sheet gets you names. A registration form gets you a roster.

What waivers and medical fields do I need?

At minimum: medical and allergy notes, an emergency contact, the adults allowed to pick up from a drop-off class, a liability waiver signed by a parent or guardian, and a photo release. What a waiver has to say varies by state and by activity, so check your state's requirements.

How do I collect payment with the form?

Collect payment in the same place as the form. A sheet that collects names and promises payment details later gives every family a week to change their mind. Common mistakes

Collecting payment separately

"Sign up here, I'll send payment info later" turns eight yeses into five students. Put the price and the payment on the form, so a signup is a seat paid for.

No pickup authorization

In a drop-off class, the adult at the door at 11:00 isn't always the adult who dropped off at 10:00. Ask who can pick up, even for a one-hour class.

Asking for the same info every season

Your best families take class after class. If every new session starts with the same ten questions, you're taxing your regulars. On enrops, families get a parent account with their first registration.

A sign up sheet with no price on it

Interest isn't enrollment. A clipboard at the front desk collects names and first-name-only handwriting. A form with the price on it collects students.

Can I use this template for free?

What's the difference between a sign up sheet and a registration form?

A sign up sheet collects names. A registration form collects everything you need to run the class: the session choice, contacts, medical notes, a signed waiver and the payment. If money or a child's safety is involved, use a registration form.

How long should a class registration form be?

Short enough to finish on a phone in a few minutes. Ask only for what you'll use this session. On enrops, a parent finished a real registration in about 1.5 to 4.5 minutes in our July 25, 2026 production test.

Can I take registrations for several classes at once?

Yes. Build each class as its own program with its own schedule, price and roster. Families pick the class that fits, and each roster fills on its own.

Do I need a lawyer to write my waiver?

We can't tell you what your waiver has to say, and this page isn't legal advice. enrops starts you with a waiver you can edit into your own words. Check your state's requirements, and ask your insurance carrier or an attorney to review the wording.

How long does it take to build this class registration form on enrops?
