What should a sports camp registration form include?
A sports camp registration form should collect the player's name and date of birth, parent or guardian contacts, an emergency contact, experience level, jersey or T-shirt size, medical and allergy notes, a concussion acknowledgment where your state calls for one, pickup authorization, a signed waiver, a photo release and payment, at one link.

What waivers and medical fields do I need?
At minimum: medical and allergy notes, an emergency contact, the adults allowed to pick up, a liability waiver signed by a parent or guardian, and a photo release. Many states also have youth sports concussion laws that call for a signed information sheet. Check your state's requirements, and ask your insurance carrier what it expects.

How do I collect payment with the form?
Collect payment in the same place as the form. A camp that takes the form now and the money at check-in starts day one with a line at the table and a cash box. On enrops, families fill in the player's details, sign the waiver and pay at one link, and the money lands in your own Stripe account.

Common mistakes
Four mistakes that make day one of camp harder.
Check-in should take a name and a high five. These are the things that turn it into paperwork.
Collecting payment separately
Taking the form online and the money at check-in means day one starts with a line, a cash box and a parent who left the checkbook at home. Collect it with the form.
No pickup authorization
Players scatter the second the last drill ends. Coaches need a list of who can pick up each player, filled in by the parent at registration.
Asking for the same info every season
The family that came to winter clinic, spring break camp and summer camp has typed the same emergency contact three times. Keep what didn't change. On enrops, families get a parent account with their first registration.
Learning about the inhaler at the first water break
Medical notes that live in an email thread don't reach the coach on field 3. Collect them on the form so they sit on the roster next to the player's name.

Sports camp registration form questions
Can I use this template for free?

Do I need a concussion form for a sports camp?
Many states have youth sports concussion laws, and the rules differ on which programs they cover. Include a concussion acknowledgment on your form if your state or your insurance carrier calls for one. Check your state's requirements.
Should I ask for a sports physical?
Only if your program, your facility or your insurer asks for one. If you do, say so before families pay and tell them how to send it. Every camp should collect medical and allergy notes and an emergency contact at registration.
How do I group players by skill?
Ask experience level on the form with three or four plain choices, then sort your roster by it. Plain choices work better than a 1 to 10 rating, because every parent's kid is an 8.
Do I need a lawyer to write my waiver?
We can't tell you what your waiver has to say, and this page isn't legal advice. enrops starts you with a waiver you can edit into your own words. Check your state's requirements, and ask your insurance carrier or an attorney to review the wording.
