Season registration is the busiest week a club or program has. Every family at once, one deadline, and a volunteer sorting age groups in a spreadsheet at midnight. This checklist covers player and guardian details, age group, sizes, the concussion acknowledgment, volunteer interest, waivers and fees, so the form does the sorting.

What should a youth sports registration form include?

A youth sports registration form should collect the player's name and date of birth, age group or division, parent or guardian contacts, an emergency contact, experience level, jersey size, medical and allergy notes, a concussion acknowledgment where your state calls for one, a signed waiver and code of conduct, a photo release and the season fee.

What waivers and medical fields do I need?

At minimum: medical and allergy notes, an emergency contact, a liability waiver signed by a parent or guardian, a photo release and a code of conduct. Many states also have youth sports concussion laws that call for a signed information sheet. Check your state's requirements, and ask your league and your insurance carrier what they expect.

How do I collect payment with the form?

Collect the season fee in the same place as the form. A registration night with paper forms and checks means someone types it all in afterward, and someone else chases the families who owed a balance. Common mistakes

Four mistakes that turn registration week into a second job.

Most youth sports programs run on volunteers and evenings. The form should give those evenings back.

Collecting payment separately

Paper forms and checks at registration night means someone types every form in afterward, and someone else chases the balances. Take the form and the fee together.

No pickup authorization

Practice ends at dusk in a public park. Coaches need to know which players leave with a named adult and which have permission to walk or ride home.

Asking for the same info every season

Spring season, fall season, winter skills. Same player, same parent, same allergy. Keep what didn't change and ask for the new size. On enrops, families get a parent account with their first registration.

Sorting age groups by hand

If the form shows the birth-date range next to each division, parents sort themselves. Hand-sorting a season's worth of players is how a nine-year-old lands on the wrong roster.

Can I use this template for free?

Do I need a concussion form for youth sports?

Many states have youth sports concussion laws, and they differ on which programs they cover and who signs. Include a concussion acknowledgment if your state, your league or your insurance carrier calls for one. Check your state's requirements.

Should I verify a player's age?

Collect date of birth on the form. Whether you ask for a birth certificate or other proof depends on your league's rules. If you do, tell families at registration how and when to show it.

Does enrops build teams and game schedules?
