Summer camp registration form template

Families start looking for summer camp months ahead, and they sign up from a phone. This checklist covers what a day camp or specialty camp needs to ask: the weeks they want, swim ability, T-shirt size, medical and pickup details, waivers and payment. What should a summer camp registration form include?

A summer camp registration form should collect the camper's name and date of birth, parent or guardian contacts, an emergency contact, the weeks or sessions they want, swim ability, T-shirt size, medical and allergy notes, who is allowed to pick up, a signed waiver, a photo release and payment. Ask for each thing once, and only what you'll use.

What waivers and medical fields do I need?

At minimum: medical and allergy notes, medications, an emergency contact, the adults allowed to pick up, a liability waiver signed by a parent or guardian, and a photo release. Camps often add sunscreen and water permissions. Licensed camps can have more to collect, so check your state's requirements before you publish.

How do I collect payment with the form?

Collect payment in the same place as the form. When the form and the payment live in two tools, some families finish one and skip the other, and you spend June matching names to deposits. Common mistakes

Four mistakes that cost camps registrations.

Every one of these is a systems problem. Systems can be fixed.

Collecting payment separately

A form in one tab and a payment link in another gives every family two chances to stop. The ones who finish the form and forget the payment become your follow-up list in May.

No pickup authorization

On a Friday at 3:00, a counselor you hired in June is deciding whether this adult can take this child. Give them a list that was filled in by the parent at registration.

Asking for the same info every season

A family booking their third summer with you shouldn't type their address again. Keep what didn't change and ask about what did: the weeks, the medical notes, the pickup list. On enrops, families get a parent account with their first registration.

One form per week

A family that wants weeks 2, 5 and 7 shouldn't start from zero three times. Let them choose every week in one sitting, with one payment or one payment plan.

Can I use this template for free?

When should summer camp registration open?

Earlier than feels natural. Families plan summer months ahead, and the camps with dates and prices posted are the ones that make it onto the calendar. Post your weeks as soon as your dates and price are set, even if details like field trips come later.

Should I offer a payment plan for camp?

A payment plan helps most when a family is booking several weeks at once. On enrops, families can split a registration into the installments you set, with no interest, plan fee or late fee.

Do I need a health form for summer camp?

It depends on your state and whether your camp is licensed. At minimum, collect allergy and medical notes, medications and an emergency contact at registration. Some states ask licensed camps for health histories or immunization records, so check your state's requirements.

Do I need a lawyer to write my waiver?
