Can I copy these templates for free?

Are these legal forms?

No. They're checklists of what to ask, from people who run kids programs. What a waiver or medical form has to say varies by state and activity. Check your state's requirements, and ask your insurance carrier or an attorney to review your wording.

Questions about registration form templates

What should every kids program registration form include?

The child's name and date of birth, parent or guardian contacts, an emergency contact, medical and allergy notes, the adults allowed to pick up, a signed waiver, a photo release, payment and your cancellation terms. Each template adds the fields its program type needs.

Do I need a different form for every program?

You need one core and a few fields that change. A camp asks about weeks and swimming. An after-school program asks about the homeroom teacher and the after-class plan. Start from the template closest to how you run, and cut what you won't use.

What does it cost to build these on enrops?
