The checklist, part 1
What should an after school program registration form include?
An after school program registration form should collect the child's name, school, grade and homeroom teacher, parent or guardian contacts, an emergency contact, medical and allergy notes, how the child gets to class and how they go home, the adults allowed to pick up, a signed waiver, a photo release and payment.

What waivers and medical fields do I need?
At minimum: medical and allergy notes, an emergency contact, the after-class plan, the adults allowed to pick up, a liability waiver signed by a parent or guardian, and a photo release. Your school partner or district may ask for more. Check your state's requirements and your facility use agreement.

How do I collect payment with the form?
Collect payment in the same place as the form. A flyer that says register here and pay there loses families between the two steps, and it leaves you matching payments to names the night before class. Common mistakes
Four mistakes that show up at dismissal.
After-school programs run inside someone else's building. The form is where you earn the front office's trust.
Collecting payment separately
A Google Form for the signup and a payment app for the money means a spreadsheet to match the two, and three reminder texts to one family about a $45 balance.
No pickup authorization
"Who's picking up today?" answered from memory at 4:28 PM is the riskiest minute of your week. Collect the pickup list and the after-class plan on the form, for every child.
Asking for the same info every season
Fall, winter, spring. Same child, same school, same allergy. Ask for what changed, and keep what didn't. On enrops, families get a parent account with their first registration.

Can I use this template for free?

Why ask for the homeroom teacher?
Because that's how a school finds a child. When a parent calls the office at 2:50 to change pickup, the office calls the classroom. A roster with grade and homeroom teacher lets your instructor and the front office work from the same list.
Should families fill in a new form every term?
Ask for what changed, and keep what didn't. A new term needs a new class choice, a new payment and a fresh look at pickup and medical notes. It shouldn't need the child's birthday again. On enrops, families get a parent account with their first registration.
What do schools want from an outside program's roster?
Front offices usually ask for names, grades, homeroom teachers and the after-class plan for each child, before the first session. Ask your school contact what they need and collect it on the form, so you aren't chasing it later.
Do I need a lawyer to write my waiver?
