What should a dance class registration form include?
A dance class registration form should collect the dancer's name and date of birth, class and level, parent or guardian contacts, an emergency contact, medical and injury notes, a dress code acknowledgment, a plain note about recital and costume fees, pickup authorization, a signed waiver, a photo and video release, and tuition payment with a payment plan option.

What waivers and medical fields do I need?
At minimum: medical and injury notes, an emergency contact, the adults allowed to pick up, a liability waiver signed by a parent or guardian, and a photo and video release, since recitals get filmed. What a waiver has to say varies, so check your state's requirements.

How do I collect payment with the form?
Collect tuition in the same place as the form. When registration is a form and tuition is an invoice that follows, the gap between the two is where families drift. On enrops, families pick the class, sign the waiver and pay at one link, and the money lands in your own Stripe account.

Common mistakes
Four mistakes that cost studios families.
Dance parents stay for years when the money side feels fair. These are the moments it stops feeling that way.
Collecting payment separately
Registration on a form, tuition on an invoice, costume fees in an envelope. Three systems means three places for a family to fall behind, and you chasing all three.
No pickup authorization
The five-year-olds in Creative Movement can't walk out to the parking lot to find their ride. Ask who can pick up, and keep the list where the teacher can see it.
Asking for the same info every season
A dancer who has been with you since age four shouldn't re-enter her birthday every August. Keep what didn't change and ask about what did: the class, the level, the sizes. On enrops, families get a parent account with their first registration.
Recital fees that show up in March
A family that budgeted for tuition and meets a costume fee mid-season feels ambushed, even when the amount is fair. Put the recital fee and the costume range on the registration form.

Can I use this template for free?

Should recital fees be on the registration form?
Yes, as a plain note, even if you collect them later. State the recital fee, the costume cost range and the due dates. Families handle a cost they saw at registration far better than one they meet mid-season.
Can families pay dance tuition in installments?
On enrops, a family can split a registration into the installments you set, with no interest, plan fee or late fee. A payment plan covers one registration, so set tuition by semester or session.
How do I place new dancers in the right level?
Ask for years of experience and the last studio on the form, then confirm placement in the first class. Say on the form that the teacher makes the final call, so a move between levels is expected.
Do I need a lawyer to write my waiver?
We can't tell you what your waiver has to say, and this page isn't legal advice. enrops starts you with a waiver you can edit into your own words. Check your state's requirements, and ask your insurance carrier or an attorney to review the wording.
