How to use this in 10 minutes

Print a stack of the form (pages 2 and 3) for every site clipboard. Read the objective-language guide once; it’s the part that makes the form work. Decide your minor-vs-serious routing (page 5) and write your two names in it. Tell instructors where blank forms live and where completed ones go.
